It is the member’s responsibility to verify that the provider is in network with their insurance plan prior to scheduling an appointment. Provider is not responsible if you are seen and not in network at the time of the visit.
Health Share plans are NOT health care or insurance plans. Currently, we do not accept patients with these health plans. Common health share plans are listed below.
Self-Pay/Cash is available to those without insurance at a reduced fee schedule. All self-pay/cash patients must pay IN FULL at time of service.
