Understanding your insurance coverage shouldn’t be complicated. Our team is here to help you understand your benefits, estimated out-of-pocket responsibility, and available payment options before treatment whenever possible.
Because benefits and network participation vary by individual plan, we encourage patients to contact our office with any coverage questions.

Insurance Verification
We will verify available insurance benefits when possible before your visit or procedure. Benefit verification is not a guarantee of payment by your insurance plan.
Understanding Your Costs
Your out-of-pocket responsibility may include deductibles, copays, coinsurance, non-covered services, or other amounts determined by your individual insurance plan.
Surgery & Separate Charges
Surgical care may involve separate charges for the physician, facility, anesthesia, imaging, durable medical equipment, or other services depending on where your procedure is performed.
Payment Options
We accept cash, check and all major credit cards. We also offer Cherry financing an Care Credit for eligible current and upcoming services for patients who prefer to divide their cost into payments.
Do You Accept My Insurance?
Insurance participation and benefits can vary significantly between plans, even when two patients have coverage through the same insurance company.
Our team can help verify your insurance benefits and provide information regarding your estimated financial responsibility before your visit or procedure whenever possible.
To help us provide the most accurate information, please have your current insurance card available when scheduling your appointment.
Before Your Visit
We recommend that patients:
Confirm that their insurance information on file is current.
Notify us of any changes to insurance coverage as soon as possible.
Contact their insurance carrier directly if they have questions regarding network participation, deductibles, copays, coinsurance, or plan-specific coverage.
Obtain any required referral or authorization prior to the appointment when required by their plan.
Please note: Verification of insurance benefits is not a guarantee of coverage or payment. Final benefits and patient responsibility are determined by your insurance carrier after your claim is processed.

Understanding Your Estimated Cost
Before certain visits, procedures, or surgeries, our team may provide an estimate of your expected out-of-pocket responsibility based on the insurance information available to us at that time.
Your estimated responsibility may include your deductible, copayment, coinsurance, non-covered services, or other patient responsibility under your individual health plan.
Why Is It an Estimate?
Insurance benefits can change throughout the year as deductibles are met, claims are processed, or coverage information is updated. Because of this, the amount provided before treatment is an estimate and may differ from the final amount determined by your insurance carrier.
Your insurance company makes the final determination regarding coverage and patient responsibility when the claim is processed.

If you receive an estimate from our office and have questions about how the amount was calculated, please contact us before your scheduled service. We are happy to review the information we received from your insurance carrier with you.
Surgery & Separate Bills
If you are scheduled for surgery, you may receive separate bills from more than one provider or facility involved in your care.
Depending on where your surgery is performed and the services you receive, separate charges may include:
Surgeon / Professional Fee
Surgery Center or Hospital Facility Fee
Anesthesia Services
Imaging or Laboratory Services
Durable Medical Equipment
Other Independent Providers or Services
These charges may be billed separately because each provider or facility may process claims independently with your insurance company.
Your Surgical Estimate
When possible, our office will provide an estimate for the services we are responsible for billing. Charges from hospitals, anesthesia groups, outside imaging centers, laboratories, or other independent providers may not be included in the estimate you receive from our office.
If you are unsure which charges are included in your surgical estimate, please contact our team before your procedure and we will help clarify what is being billed by our office.
Self-Pay & Cash-Pay Services
We offer self-pay options for patients who do not have insurance coverage for a service, choose not to use insurance, or receive treatments that are not covered by insurance.
Whenever possible, our team will provide the expected cost before treatment so you can understand your financial responsibility in advance.
Important
Self-pay pricing applies only to the services specifically included in the quote or estimate provided by our office. Additional services from outside providers, facilities, anesthesia groups, imaging centers, laboratories, or other independent entities may be billed separately.
Workers’ Compensation & Other Coverage
We treat patients with approved workers’ compensation claims and other authorized third-party coverage when appropriate.
Because these cases often require additional documentation and authorization, please provide our office with the following information as early as possible:
- Employer name
- Workers’ compensation insurance carrier
- Claim number
- Adjuster or case manager contact information
- Required authorization or referral information
Any other documentation requested by our office
Treatment, imaging, procedures, and surgery may require authorization before services can be scheduled.

Payment Plans & Financing
We do not offer in-house payment plans.
For patients who would like the flexibility to pay over time, we partner with Cherry, a third-party financing company that offers payment plan options for eligible current and upcoming services.
Cherry financing may be available for:
Upcoming surgical expenses
Professional fees
Eligible surgery center expenses
Self-pay procedures
Regenerative therapy
Other eligible current treatments
Patients can review available payment options and decide whether Cherry financing is right for them.
NOTE: Cherry financing cannot be used for past-due balances or previously billed services.
